NAA Guardian .32 questions

I picked this up USED, and well aware of the many issues.  My main concern is I took it apart as soon as I brought it home.  I cleaned it up and applied some light gun grease on a few spots.  I noticed that the bolt holding the cover on the left side was loose, so I tightened it up.  I noticed that cover was loose before I tightened the screw, then it was very snug.  I took it out to shoot and every other shot didn't fire.  It always fired on second trigger pull.  It was a bang click bang click bang for both magazines  Back inside and I took panel off and slightly loosened that cover screw (bolt) and back out and applied a light coat of oil under that cover and removed the firing pin and spring and cleaned the firing pin and channel.  Two mags through it with not a single hiccup. I also noticed the face of the hammer is bowled in a little.....Not sure if this is causing my light hammer strikes either?  I did get two full mags through it with no problems after the oil, cleaning and loosening that screw a little.  I'd rather not have that screw loose though.  Is it supposed to be tight to hold that cover tight against the trigger arm?
